A task force is looking into the death of Ann Marie Smith, who was forced to live in disgusting and degrading conditions while under the care of the National Disability Insurance Scheme.

The 54-year-old died in hospital in April from septic shock, multiple organ failure, severe pressure sores and malnourishment.

Police have launched a manslaughter inquiry into her treatment and the NDIS commissioner has appointed former Federal Court Judge Alan Robertson to lead an independent inquiry.

Ms Smith was remembered in an online vigil on Saturday, with people lighting candles and leaving messages of solidarity.

Advocate Samantha Connor, who herself lives with a disability, spoke at the memorial and said Ms Smith's death was about more than just service failures.

"We know that this happens to disabled people every day. This is why we have a royal commission that's on hold right now," she said.

"They essentially boil down to the same sorts of issues and they're not just around service failures. They're around things like loneliness and isolation and ableism, people not wanting to have anything to do with disabled people."

Ms Connor said while preventative safeguards such as police checks were important, it was crucial that the community also did its part.

"Just the neighbours being able to see that she hasn't left the house and to just drop in and say 'is everything OK?'," she said.

"I think we have a real opportunity right now that people who aren't disabled are understanding, perhaps for the first time, what isolation and segregation looks like because they're in the middle of a pandemic and they're living the life that so many of us live right now."

More than a third of people involved in SA's disability sector who were surveyed this week said Ms Smith's death came as no surprise, while 60 per cent believed the instance of physical or mental harm caused by people caring for others was a regular occurrence.
